If your child requires medication to be administered during nursery hours on a short term basis ie antibiotics please print out and complete the short term form. If you wish medication to be administered on a long term basis, please print out and complete the long term form. The completed forms should be handed into the school office along with the medication which should be clearly labelled with the child's name.
Please note the first dose of medication must be administered by parents to ensure your child does not take an allergic reaction to the medication.
Please note Nursery staff cannot administer prescribed medication to any child without prior written consent from the parent/carer.
