There are new forms to complete should your child require medicine to be administered during the school day. This applies to prescribed and non-prescribed medication.

Please print off a copy of Form 9/06/F01 (below). The completed form should then be brought in to the school office by an adult, along with the medication to be administered.

Points to note

  • School staff cannot administer prescribed or non-prescribed medication to any pupil without Form 9/06/F01
  • Medication will only be accepted in the original packaging where it states:
    • The prescribed pupil’s name
    • Name of medication
    • Instructions for its administration to the named pupil
  • Should a pupil require two or more medicines, each medicine must be in a separate container containing the above details
  • An adult must always be responsible for bringing any medication to school and for collecting it at the end of the school day. School staff will not return medication to a child.
  • Medication (prescribed or non-prescribed) should not be carried by a child in school e.g. kept in their school bags, in trays etc
